However the optimum outcomes with preservation of vision is most likely to be achieved with early detection, i.e. at the time of onset of nAMD before fibrosis and atrophy can supervene and destroy the macular retina permanently. Although patients with nAMD can become symptomatic at the time of onset, a large proportion may miss the symptoms particularly when the first eye is involved as the second eye can compensate. Even when the second eye develops nAMD patients may not report these symptoms particularly if other pathology such as cataract is present.
